請於 開辦課程前兩星期(視乎課程所需) 將以下資料交回本會秘書處：

郵寄地址：九龍新蒲崗大有街34號新科技廣場28樓21室中國香港龍舟總會
1) 已填妥開辦課程申請表格；

 

課程完結後，需將填妥之「申領證書表格」及所需費用交回本會秘書處。
劃線支票抬頭請書寫『中國香港龍舟總會』，並於支票背面註明︰屬會名及聯絡電話。

The Certificate Issue Form should be completed and submit to the Secretariat with the payment.

Crossed cheque made payable to “Hong Kong China Dragon Boat Association”, please write down the club's name, contact number and course on the back of the cheque.
