Application methods and qualification of registration are list as follow.

 

所有註冊/更新註冊請下載表格，填妥後連同表格正本、所需費用及文件副本 寄回本會秘書處，

個人會員註冊適用  Suitable for Individual Member Registration 

​

申請資格：歡迎任何人士申請成為本會個人會員。

 

申請方法：

請將以下資料寄交本會秘書處。

1.  填妥之申請表

2. 旅行證件相片 (40mm x 50mm)

3. 申請費用，只接受支票或銀行入數紙  ( 新註冊或更新註冊$30/1年 )

4. 貼上$2 郵票之回郵信封 ( 如需寄回收據 )

 

 

Qualification: Welcome all person to apply as our Individual Member.

 

Application Method:

Please mail the documents stated below to the Secretary.

1. A filled application form.

2. Photograph for Travel Document (40mm x 50mm)

3. Application Fee, either by Cheque or Deposit Receipt (New Member or Renewal $30/ year)

​4. Reply Envelope with a HKD $2 Stamp (Necessary if you need a receipt)

教練/舵手註冊適用 Suitable for Coach/Helm Registration 

​

申請資格：已持有本會發出之教練證書者可申請成為本會註冊教練。

​

申請須知：

1.  凡新註冊之教練，均會獲發註冊教練制服，

2. 註冊教練無需再註冊成為註冊舵手，註冊教練身份已包括註冊舵手

​

申請方法：

請將以下資料寄交本會秘書處。

1.  填妥之申請表

2. 申請費用，只接受支票或銀行入數紙  ( 新註冊$300/年；更新註冊$100/年 )

3. 旅行證件相片 (40mm x 50mm)

4. 香港身份證副本

5. 無性犯罪紀錄聲明書

6. 以下機構發出的有效之急救証書副本

     -  香港聖約翰救護機構；或

     -  香港紅十字會；或

     -  香港醫療輔助隊

7.  填妥之個人會員申請 ( 所有註冊教練者必須為本會有效會員 )

8.  教練證書副本 (新註冊適用)

9. 3年內的教練研討會或工作坊證書

10.  貼上$2 郵票之回郵信封 ( 如需寄回收據 )

Qualification: Welcome all holders of HKCDBA Coach Certificate to registrate as our Registrated Coach.

Application Method:

Please mail the documents stated below to the Secretary.

1. A filled application form.

2. Application Fee, either by Cheque or Deposit Receipt (New Registration $300/year; Renewal $100/year)

3. Photograph for Travel Document (40mm x 50mm)

4. HKID Card (Copy)

5. Declaration of No Criminal Conviction

6. First-Aid Certificate issued by the agency / organization below:

    - Hong Kong St. John Ambulance; or

    - Hong Kong Red Cross; or

    - Auxiliary Medical Service

7. A filled Individual Membership form (All registrated coaches are required to be our Associate Member)

8. HKCDBA Coach Certificate (For new registration only)

9. Certificate of Coaching Workshop or Seminar within 3 years

10. Reply Envelope with a HKD $2 Stamp (Necessary if you need a receipt)

裁判註冊適用 Suitable for Race Official Registration 

 

申請資格：已持有本會發出之裁判證書者可申請成為本會註冊裁判。

 

申請方法：

請將以下資料寄交本會銅鑼灣秘書處。

1.  填妥之申請表

2.  申請費用，只接受支票或銀行入數紙  ( 新註冊$300/1年；更新註冊$70/1年 )

3.  旅行證件相片 (40mm x 50mm)

4. 正本實習紀錄卡(新註冊適用)

5. 裁判證書副本 (新註冊適用)

5.  填妥之個人會員申請 ( 所有註冊裁判者必須為本會有效會員 )

6.  貼上$2 郵票之回郵信封 ( 如需寄回收據 )

 

 

Qualification: Welcome all holders of HKCDBA Race Official Certificate to registrate as our Registrated Race Official.

 

Application Method:

Please mail the documents stated below to the Secretary.

1. A filled application form.

2. Application Fee, either by Cheque or Deposit Receipt (New Registration $300/year or Renewal $70/year)

3. Photograph for Travel Document (40mm x 50mm)

4. Original Copy of Practice Record Card (Appropriate for New Registration)

5. A filled Individual Membership form (All registrated ROs are required to be our Associate Member)

6. Reply Envelope with a HKD $2 Stamp (Necessary if you need a receipt)
